An employee was using a metal rod to dislodge a grain blockage from the outside of a grain bin withdraw chute. When the blockage cleared, the rod twisted down and contacted the drag conveyor below. The rod was then pushed against the chute and pinched the employee's hand resulting in amputation of the ring finger and a contusion on the middle finger.

On or about November 10, 2015, an employee was greasing the drive shaft on the end of a conveyor. The employee's sleeve got caught in the conveyor, and the employee's hand was pulled into the drive shaft. The employee's fourth and fifth left-hand digits were lacerated and fractured.
